for qualified patients.
Dr. Brett Levine is a Qualified Physician under Florida law to certify Qualified Patients to receive medical marijuana.
Are you a candidate for medical marijuana?
Contact LevMed to set up an appointment. During your visit, Dr. Levine will determine if you are qualified to receive medical marijuana. Under Florida law, you (the patient) must present with at least one of the following conditions:
Qualifying Medical Conditions:
Cancer
Epilepsy
Glaucoma
HIV
AIDS
PTSD (Post-Traumatic Stress Disorder)
ALS (Amyotrophic Lateral Sclerosis)
Crohn's Disease
MS (Multiple Sclerosis)
Conditions of the same kind or class
Chronic nonmalignant pain (caused by or originating or persisting from a condition listed above)
A terminal condition (diagnosed by a physician other than the physician issuing the order)
Dr. Levine will register your medical information in the secured Medical Marijuana Use Registry. After, you can apply for and receive an Identification Card.
Dr. Levine can help you contact a state-approved medical marijuana treatment center (MMTC) to fill your medical marijuana order.
